填写开户表格须知

1 Language 填表所用语言

Name and address must be written in English or Chinese. 姓名及地址必须以英文或以中文填写。

2 Submission 提交方式

Application Forms can be submitted in person together with their original copy of identity card or passport and proof of address. 申请人可以把本表格亲自交回本公司。提交本表格时，必须带备身份证明文件及地址证明文件以便本公司核实身份。

Application Forms can also be submitted by post together with a certified photocopy of proof of identity and proof of address 申请人也可以把本表格连同经认证的身份证明文件副本及地址证明文件副本邮寄到本公司。

Application Forms can also be submitted by FAX or EMAIL the Scan Copy to the appointed NO. or Email.

申请人也可以把本表格连同经认证的身份证明文件副本及地址证明文件副本传真或电邮至指定传真号码或电邮地址。

3 Proof of Identity and Address 身份及住址证明

All applications must be accompanied by (1) A photocopy of proof of identity, and (2) Proof of address. 所有的开户表均必须附上一份身份证明文件副本及一份住址证明文件。

Proof of identity includes passport, identity card, driving licence and any other documents with a similar nature. 身份证明档包括护照，身份证，驾驶执照或其它具有相似性质之文件。

Proof of address includes bank statements, utility bill and telephone bills etc. The address appearing on the address proof should be same as taddress provided in this Form. 住址证明文件包括银行月结单，水电费单和电话费单等。住址证明文件上面的住址必须与填写在本表格里的地址相同

Certification means a document certified by a lawyer, accountant, bank manager, persons or corporations licensed by or registered with a regulatory body. 经认证的身分证明文件及住址证明文件副本表示该等档经由律师，会计师，银行经理，持牌的证券商，持牌的证券中介人或政府证部门签署 认证。

4 Password 密码

Applicants must provide a password to be used to log into the trading platform. The password can be a combination of numerical number or/aalphabetic characters. Password must be kept in a safe place.

申请人必须提供一个 4-8 位数字，用作登录交易平台的密码。该密码可以是数字元或/及英文字母的组合。登录密码必须存放于安全的地方。 5 Joint Account Holder Name 联户账户持有人
